Understanding DOT Physical Requirements for Commercial Drivers

Commercial motor vehicle (CMV) operators are the backbone of American commerce and supply chains. To ensure safety across our highways, the Federal Motor Carrier Safety Administration (FMCSA) mandates that all commercial drivers undergo regular Department of Transportation (DOT) physical examinations.

A DOT physical examination evaluates whether a driver meets the physical and psychological requirements to safely operate commercial vehicles. Core medical standards evaluated include:

1. Vision Screening: Drivers must have at least 20/40 visual acuity in each eye separately and both eyes together (with or without corrective lenses) and at least 70 degrees of peripheral vision in the horizontal meridian in each eye.

2. Hearing Acuity: Drivers must perceive a forced whisper in the better ear at a distance of not less than 5 feet, or meet specific audiometric testing standards.

3. Blood Pressure & Cardiovascular Health: Blood pressure must fall within acceptable FMCSA stages (ideally under 140/90 mmHg for a full 2-year certification). Drivers with hypertension can be certified with appropriate medical control.

4. Dipstick Urinalysis: Evaluates protein, blood, and glucose levels to screen for underlying kidney disease or unmanaged diabetes.

5. Physical Examination & Medical History: Evaluates neurological function, musculoskeletal range of motion, respiratory health, and compliance for drivers managing sleep apnea (CPAP compliance logs) or insulin-treated diabetes.
