How Online Registration Works

Choose between urgent care walk-in check-in (Clockwise.MD) or primary care scheduling with Dr. Soleye (Healow).
Complete Digital Intake
All demographic, medical history, emergency contact, and consent forms are embedded directly in the registration links.
Arrive for Your Care
Present your photo ID and insurance card upon arrival. Your digital chart will already be prepared for the medical team.
- Valid government-issued photo ID (Driver's License, Passport, State ID)
- Current medical insurance card (if using insurance)
- List of current medications, dosages, and prescribing doctors
- Method of payment for any applicable co-pay or self-pay visit fee
