How to Prepare for Your Initial Appointment: Complete your new patient intake and medical history forms online in advance. Bring a valid government-issued photo ID, your active insurance card, immunization records, recent diagnostic records or lab reports from previous providers, and a complete list of current prescription and over-the-counter medications.
What Happens During Your First Visit with Dr. Soleye: 1) Thorough Clinical History Review: Dr. Soleye will review your past medical conditions, surgical history, family health risk factors, and personal health goals. 2) Baseline Vital Signs & Physical Assessment: Our nursing staff evaluates blood pressure, pulse, temperature, oxygen saturation, and body metrics, followed by a head-to-toe examination. 3) Comprehensive Baseline Lab Testing: When indicated, routine blood work (CBC, metabolic panel, lipid panel, thyroid panel) is completed promptly on-site. 4) Collaborative Care Planning: Before leaving, you and Dr. Soleye will co-create a clear preventive healthcare plan, including any necessary prescription adjustments or specialist referrals.
